Clinical Pharmacist-Home Care

Job Summary: This position will support LLUMC's strategic plan and the organization's mission to continue the teaching and healing ministry of Jesus Christ, while embracing the core values of Compassion, Integrity, Excellence, Teamwork, and Wholeness. Reports to Pharmacy Supervisor-Mt. View Pharmacy. Through coordination with the Home Health staff, performs pharmaceutical patient assessment (initial and reassessments), develops plans to manage the patient's pharmaceutical care and executes the care plan. Participates in pharmaceutical services related to the preparation and dispensing of drugs and other pharmaceutical supplies to patients at home. Promotes rational drug therapy to physicians, optimize neonatal, pediatric and adult patient outcomes, and minimize home care costs whenever possible. Complies with all applicable state, federal, local laws, rules and regulations as well as Joint Commission standards. Performs other duties as needed. Job Specifications: Ability to perform pharmaceutical assessments and reassessments, creating and executing plans to manage the pharmaceutical care of home care patients and overseeing interns and technicians required. Able to read; write legibly; speak in English with professional quality; use computer and software programs necessary to the position; troubleshoot and calibrate patient care equipment. Able to relate and communicate positively, effectively, and professionally with others; be assertive and consistent in following and/or enforcing policies; work calmly and respond courteously when under pressure; lead, supervise, teach, and collaborate; accept direction. Able to communicate effectively in English in person, in writing, and on the telephone; think critically; work independently with minimal supervision; perform basic math functions; manage multiple assignments effectively; work well under pressure; problem solve; organize and prioritize workload; recall information with accuracy; pay close attention to detail. Graduated from school of pharmacy accredited by the American Board on pharmaceutical education. PharmD. Degree preferred. Minimum of two years of Clinical pharmacy experiencerequired. Minimum of two years of Home Infusion experiencepreferred. Current California pharmacist license required.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
